If you complete an Adult Admissions or Adolescent Admissions form through our Site, the information you provide is transmitted through Formstack's platform to our clinical and administrative staff for the purpose of initiating care. While this information is being collected prior to the formal start of a treatment relationship, we treat it with the same care and confidentiality standards we apply to patient health information generally.

Once you become a patient, all clinical records and communications regarding your care are governed by our HIPAA Notice of Privacy Practices and, for substance use treatment services, by 42 CFR Part 2, which provides additional federal confidentiality protections for records related to substance use disorder treatment.

7. Data Security

We use reasonable administrative, technical, and physical safeguards designed to protect information submitted through our Site. However, no method of transmission over the internet or electronic storage is 100% secure, and we cannot guarantee absolute security.

8. Children's Privacy

Our Site includes information about services for adolescents and children (such as play therapy and adolescent admissions).
